THE SERIOUS STUFF
Although accidents are rare, they do happen. With this in mind, it is essential that all participants (and the parents/guardians of participants under 18) understand that they are about to take part in an extreme sport. All participants must read and acknowledge (by providing a signature) the below statement. The lead booker has the following options available to them:

Tick the 'assume responsibility for all participants' box within the form below. This must only be used when the lead booker has the authority to do so, such as booking for family and externally insured corporate groups etc.

Print off the acknowledgement form (located here) & have it completed prior to the start of the session, where it will be presented to a member of Zorb Wars staff.

Advise all participants that they will be required to sign the form on-site prior to the session start. A session cannot begin until all participants have signed the form.
'THE STATEMENT'
I UNDERSTAND THAT I AM ABOUT TO PARTICIPATE IN AN ‘EXTREME SPORT’ AND HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THIS ACTIVITY.
I waive, release and discharge Zorb Wars/Kick-Off Sports Centre, their directors, board members, officers, employees, volunteers, agents & representatives from any and all liability including (but not limited to) liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may occur to me as a result of participation in the above named activity. I agree to indemnify, hold harmless, and promise not to sue the entities or persons mentioned above from any and all liabilities or claims made as a result of participation in this activity. This accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
I CERTIFY THAT I HAVE READ THIS STATEMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
